By participating in programs, services, events, and activities of The M.E.A.L. 2 Chew On Summit you agree to abide by all procedures established to protect attendees and staff, including temperature taking, wearing a mask, social distancing, and the following:

On behalf of yourself and minor children (under 18 years of age), you hereby release, discharge, and hold harmless The M.E.A.L. 2 Chew On Su, its employees, agents, and representatives, of and from all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to your participation in our programs, services, events or activities.

You understand and agree that this release includes any claims based on the actions, omissions, or negligence of this organization, its employees, agents, and representatives, whether a COVID infection occurs before, during, or after participation in any such program, service or activity.

By participating in programs, services, events, and activities of The M.E.A.L. 2 Chew On Summit, you agree to abide by all procedures established, protect attendees and staff, including temperature taking, wearing a mask (a mask must be worn under a face shield), social distancing, and the following:

I acknowledge and agree that, in the past 10 days, neither me and/or my children have:

• Tested positive of COVID in the past 10 days
• Had contact with anyone confirmed to have COVID
• Fever over 100 degrees within the last 72 hours (temperatures over 99.5 will not be able to attend)
• Cough, sore throat and/or runny nose
• Shortness of breath or difficulty breathing
• Chills/Repeated Shakes w/Chills
• Headache and/or Muscle Pain
• Loss of Taste or Smell

To ensure a safe environment, anyone experiencing any of the above symptoms will be unable to participate with us in person. you agree to abide by all procedures established to protect attendees and staff, including temperature taking, wearing a mask, social distancing, and the following:
